If you have any questions relating to travel vacinations, please fill out the Travel Health questionnaire below and either send or bring the form to the surgery. 

An appointment may be made with the nurse approximately one week later.  Please be aware that we may signpost you to a local Travel Clinic as we concentrate more on core NHS services in future. 

Please ensure that you allow sufficient time prior to your travel - allow at least 6 weeks prior to travel to submit the form for assessment, otherwise it may be too late for any vaccinations to be fully effective.

Provision of anti malarial and non-NHS vaccinations will now be directed to external travel clinics.

It is important to make this initial contact with the appropriate information as early as possible  - at least 6 weeks before you travel - Vaccines may have to be ordered as they are not a stock vaccine.  Vaccines should normally be administered at least 2 weeks before you travel to allow the vaccines to work effectively.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS.  For these you will be advised and directed to a travel clinic.
