NAMED ACCOUNTABLE GP

NAMED AND ACCOUNTABLE GP

What is the named GP requirement?   This is a contractual requirement for the surgery and builds on the 2014-2015 agreement to provide a named and accountable GP for over 75s. The named GP requirement has now been extended to all patients including children.

You will have been allocated a named and accountable GP automatically by our clinical system (usually the GP you see the most). If you wish to know, or change your named GP, please ask at Reception.

WHAT DOES 'ACCOUNTABLE' MEAN

The GP contract requires the named accountable GP to take responsibility for the co-ordination of all appropriate services required under the contract and ensure they are delivered to each of their patients where required (based on the clinical judgement of the named accountable GP).
